Background Info

Our loved ones Luke Henderson and David Ramsay asked for help a number of times before their deaths. Both men attempted to get help on a number of occasions. Our families expressed concerns to mental health staff that our loved ones were planning to take their own life. Our concerns were dismissed and no help or support was offered to help our familes. Both men went onto take their own life. We feel Luke and David were let down by the current mental health system. We feel our complaints were not listened to and we still have no answers. Things we are wanting reviewed. 1. We feel assessments tools used are inadequate and not fit for purpose. 2. A review of crisis support services outside office hours. 3. If a person dies by suicide and has been in contact with mental health services 3 month prior to their death, a fatal accident inquiry is automatically done. 4. We want to make sure departments, such as accident and emergency, community psychiatric nurses and addiction services, start to communicate with each other.
